In order to book locum shifts through Clarity Locums, you will need to make sure you have completed the necessary registration with each pharmacy you intend to work for, in order to get paid. We have set out these documents in a checklist below.

Step 1: Complete your Registration
Boots require you to accept the terms of their locum agreement and provide them with the most up to date information about yourself on your profile. To do this please complete the following actions:

This registration form will capture key information from you and also provide you with a tick box so you can confirm your acceptance of the locum agreement.
The registration form will take approximately 10 minutes to complete. You must complete the registration process fully in one session, or it will not be saved. Therefore, please ensure you have the following information to hand as you will be required to provide this information when completing your registration:
Once you have supplied this information, Boots will carry out the necessary checks and add you onto the list of locums who can provide locum services to Boots.
Step 2: Payment Set Up
In order to be set up for payment your full locum registration must first be completed above. Once you have done this, please then click on the link below to enter your payment information:
Please ensure you enter [email protected] in the box that says 'Agency Email Address' as we will need to fill in some details to complete the registration.
Following your submission of the above form you will receive an email from us to confirm your email address, please look out for this email in your inbox.
