Job Summary:
As a Cross Sector Advanced Clinical Pharmacist at Band 8A level, you will hold a senior position within the pharmacy profession, providing expert clinical pharmacy services across various healthcare settings. Your role is instrumental in optimising medication regimens, ensuring patient safety, and enhancing the quality of care in multiple sectors, such as community pharmacy, primary care, and hospital settings.
Key Responsibilities:
Clinical Leadership: Provide clinical leadership within the pharmacy team, offering expert advice and guidance to other pharmacy staff and healthcare professionals across different sectors.
Medication Review: Conduct comprehensive medication reviews for patients, optimising therapy regimens, monitoring for potential drug interactions, and ensuring evidence-based treatment in different healthcare settings.
Patient Support: Offer comprehensive pharmaceutical care to patients and healthcare professionals in various sectors, providing information on medication management, potential side effects, and drug interactions.
Medication Safety: Lead medication safety initiatives, identifying and mitigating potential risks and adverse events related to high-risk medications, regardless of the care setting.
Quality Assurance: Participate in quality assurance activities, including audits, reporting mechanisms, and the development of pharmacy policies and procedures that can be applied across multiple sectors.
Clinical Governance: Engage in clinical governance activities, including research, audits, and quality improvement projects to enhance patient care and outcomes in diverse healthcare settings.
Collaboration: Collaborate with healthcare professionals across different sectors, including community pharmacists, primary care providers, hospital teams, and other healthcare professionals, to ensure integrated and coordinated patient care.
Education and Training: Provide education and training to healthcare professionals, including pharmacy staff, junior pharmacists, and clinical staff in different care settings.
Documentation: Maintain accurate patient medication records and effectively use clinical software systems to track patient progress and medication administration across various sectors.
Requirements:
Qualifications: A Master's degree in Pharmacy or equivalent advanced qualification, along with registration with the General Pharmaceutical Council (GPhC).
Clinical Experience: Significant clinical experience, with experience across multiple healthcare sectors, including a focus on clinical pharmacy, and experience in a senior or leadership role.
Clinical Expertise: Proficiency in clinical pharmacy practices that are transferable across different healthcare settings, and a deep understanding of pharmaceutical care in various sectors.
Leadership Skills: Strong leadership and interpersonal skills, including experience in managing and leading pharmacy projects and teams in diverse sectors.
Regulatory Knowledge: Deep understanding of NHS policies and procedures, pharmaceutical regulations, and clinical guidelines applicable to various healthcare sectors.
Communication Skills: Excellent communication skills for effective collaboration with healthcare teams, patients, and stakeholders across different sectors.
Commitment to Continuous Learning: Demonstrated commitment to ongoing professional development, staying updated with industry advances, and participating in relevant continuing education to adapt to various care settings.
